Child Talent Release Form

Hi Parents and Guardians

Just like in Hollywood, students under age 18 at Babel Film Workshop require consent from their parents/guardians to participate in film and media arts projects. If you’ve already filled out this form for your child for a past course with us, or have signed and returned the printed copy, you don’t need to submit this again, unless you would like to update your responses.
