Hi Parents and Guardians! In preparation for your child’s Hollywood Hong Kong activity, please submit your child’s preferred T-shirt size and the release form below.

Just like in Hollywood, students under age 18 at Babel Film Workshop require consent from their parents/guardians to participate and appear in film and media projects. If you’ve already filled out this form for your child for a past course with us, or have signed and returned the printed copy, you don’t need to submit this again, unless you would like to update your responses.
